The pharmacist providing the weekly service completes medication reviews and attends multidisciplinary meetings. The pharmacy team has links with the community nurse teams in North and South Warwickshire to provide advice on medications and governance. At the Central England Rehabilitation Unit (CERU) the pharmacists advise on medications for complex patients receiving neurorehabilitation following stroke, brain injuries or multiple sclerosis.
Dispensing of outpatient prescriptions and certain hospital discharges takes place via the Trust’s own subsidiary called SWFT Clinical Services, situated of the main corridor within the main hospital and on the Stratford Hospital site.
