What we do
We deliver expert nursing care tailored to advise, educate and support heart failure patients and their carers, encouraging them to become active participators in their heart failure management. We also provide medication review, advice and initiate volumetric analysis of medication according to agreed guidelines and in liaison with heart failure consultants and GP colleagues.
