HomeFirst is a new health and social care service that will help you to maintain your health and daily living skills to enable you to stay or return home safely.
We understand the importance of being able to live safely at home. To support this Warwickshire County Council and South Warwickshire NHS Foundation Trust are launching a joint service called HomeFirst.
You may benefit from the HomeFirst service if you have experienced an unexpected health crisis, a change in your health which has affected your abilities to do daily living tasks either at home or recently discharged from hospital.
HomeFirst service is a short term service which assesses you at home and develops a plan with you to:
• Treat, care and support health conditions
• Provide a Therapy programme
• Arrange equipment where needed
• Refer you to other services to meet your on-going needs
How long will the service be involved?
Depending on your needs the service may only be involved for a few days but can be involved up to 6 weeks.
Is the service free?
The HomeFirst service is free. However if following treatment your assessment team feel on-going care is required, a package of care will be recommended, which may be charged for.
How do I access the HomeFirst service?
Your GP practice staff, nurse or social worker are able to refer you into the service.« Return to Adult out-of-hospital services
