There are two ways to be referred for maternity care at SWFT. The one that’s right for you depends on which GP you’re registered with.
If you are registered with a GP in South Warwickshire, you no longer need to see a GP and can refer yourself for maternity care by filling out our online form.
We’re happy to help if you have any difficulties completing the form or any other issues. You can speak to us in the Bluebell Birth Centre by calling 01926 495321 ext 6977.
To refer yourself for maternity care, please use the maternity care referral form.
We will then be in touch by email with some useful advice about keeping well in pregnancy and the details of your appointments. If you don’t receive any emails from us, please check your junk folder.
If you’re registered with a GP practice outside of South Warwickshire, you won’t be able to refer yourself directly for maternity care at SWFT. If you would like to be referred, please speak to your local midwifery team. They can complete the referral form for you and include the extra information we need.
Whilst we do our best to accommodate referrals from outside South Warwickshire, this isn’t always possible due to capacity limitations. To help us manage growing demand and ensure we can continue delivering the best possible care, we have made the difficult decision to update the referral criteria for our maternity services. Please read our full statement in the 'updated referral criteria for maternity care' tab to see if the changes apply to you.
If you are a healthcare professional submitting an out-of-area maternity referral, please use the following maternity care referral form.
At SWFT, providing safe, high-quality maternity care remains one of our top priorities.

What’s Changing?
With immediate effect, we can no longer accept late transfers of care for women and birthing people who are more than 16 weeks and 6 days pregnant at the time of referral and are registered with a GP practice outside of South Warwickshire.
Starting from 1st August 2025, we’ll be prioritising maternity referrals from people who are registered with a GP in the Coventry and Warwickshire Integrated Care System (ICS).
We’ll also continue to accept referrals from people registered with GPs within:
- Birmingham and Solihull ICS
- Banbury area (postcodes OX15, OX16, and OX17)
However, we will no longer accept referrals from people registered with GPs within:
- Herefordshire and Worcestershire ICS
- Gloucestershire ICS
- Northamptonshire ICS
- Buckinghamshire, Oxfordshire, and Berkshire West ICS (with the exception of the Banbury area)
- Any other area not listed above
What is an Integrated Care System (ICS)?
An ICS is a partnership between the NHS, local councils, and other organisations. These systems work together to plan and deliver health and care services for people in a specific region. The goal is to make healthcare more joined-up and better meet the needs of local communities.
Why Are We Making These Changes?
The number of people choosing SWFT for their maternity care has grown significantly. To ensure we maintain high standards of safety and quality, we need to manage how many people we can safely care for at any one time.
We understand this may be disappointing for some. Please know this decision will be kept under review, and we remain committed to supporting the choices of all women and birthing people as best we can.
If you’re unsure whether these changes affect you, please speak to your midwife for further guidance.
Thank you for your understanding and support.
Folic acid reminder
The NHS advises that you take a 400 micrograms or 5mg folic acid tablet every day before you’re pregnant and until you’re 12 weeks pregnant. Folic acid can help prevent birth defects known as neural tube defects, including spina bifida. If you did not take folic acid before you conceived, you should start as soon as you find out you’re pregnant. More information about the right amount for you can be found here.
