Coventry and Warwickshire Integrated Care Record (ICR)

The Coventry and Warwickshire Integrated Care Record (ICR) is a way of confidentially bringing together all your separate medical records from the different organisations involved in your health and care.

Through the ICR, health and social care professionals will be able to see relevant information about the care and treatment you've had across all services. Having a more joined-up view of a patient’s information will help to deliver better, safer care.

The benefits include:

  • Not having to repeat your details every time you have a different episode of care
  • Better and potentially faster treatment due to health and care professionals having access to better information
  • Reduction in potential medication errors through clinicians having access to your medication history (past and present)
  • More effective treatment should you need care for COVID-19 due to fast access to information about any pre-existing conditions and your medications.

What happens next?

The change is taking place automatically as we now switch on the Shared Care Record across the organisations taking part in the programme.

All patients have a right to object at any time. This could mean that information that could be vital for treatment and care – for instance, during a visit to a hospital Emergency Department – might not be immediately to hand as a result, but everyone has that right to object should they want to.
