A Rugby patient experiencing severe mobility difficulties, PTSD-related distress and declining skin health was referred to our Trust’s community nursing and therapy teams. Following a joint assessment by nurses, CRT, physiotherapists and occupational therapists, a coordinated plan was developed to support both physical needs and emotional wellbeing.

Essential equipment - including a hoist, hospital bed, specialist mattress, and commode - were installed promptly, enabling safer transfers, greater comfort, and improved wound care. Further referrals to mental health services, bladder and bowel services, social care and safeguarding ensured the patient received comprehensive, wraparound support.

Through effective collaboration across community teams in Rugby to proactively support patient health and wellbeing, the teams were able to prevent a crisis, avoid a hospital admission and make meaningful improvements to the patient’s daily life.
