Urgent community response provides a 2 hour or same day response (as appropriate to a patient’s clinical needs) to avoid an unnecessary hospital admission.
The service is for adults living in Warwickshire who are unwell and unable to access ambulatory (walk in) services due to frailty and/or a chronic long term health condition. Common conditions which can be treated include:
- Increased frailty due to illness
- New confusion or delirium
- Suspected infections such as urinary tract infections (UTIs)
- Falls with no or only minor injuries
- Urgent catheter care
- End of life crisis support
- Reduced mobility / deconditioning
- Urgent diabetes
- Urgent equipment provision
- Carer breakdown which may put the patient at risk of hospital admission.
An additional overnight service (Rapid Response) supports people at end of life and/or with blocked catheters out of hours. Eligible patients will be given the direct contact number to use for the overnight service where appropriate which operates from 8pm to 8am.
