CRT work in collaboration with the Out of Hospital community teams, South Warwickshire GP’s, West Midlands Ambulance Service, Acute Hospital within South Warwickshire and out of area trusts.
CRT is a short-term service that is in place for up to 72 hours. An initial assessment will be completed in the patient’s own home by a member of our team.
A plan will be implemented based on the patients current needs to ensure that they are supported to stay safe and well at home. Patients will receive support from our clinical support workers who will enable you to undertake day-to-day tasks, while you are recovering from a period of illness.
- We Facilitate patient to return home safely and be supported when they leave hospital.
- We Provide support to keep patients safe at home and avoid an unnecessary admission to hospital.
- We Enable and support patients with day-to -day tasks, participating in a rehabilitation approach while they recover from a period of illness.
- We Provide end of life support.
- We Support with carer breakdown.
- If patients require ongoing support, the team will complete onward referrals for example to occupational therapy, physiotherapy social services and CHC.
Any health or social care professional can make a referral to the team, providing that they have patient consent to do so.
Referral Criteria:
- Over the age of 18
- Be a patient with a South Warwickshire GP, through the integrated Single Point of Access (iSPA) 01926 600 818 Option 1.
Operating hours are from 08:30-21:30, seven days a week, including bank holidays.
