The aim of the service is to provide comprehensive care and support for adults who have a diagnosis of dementia in the community.
Our service provides individualised care planning, multidisciplinary assessments, and interventions to address physical, cognitive, and psychosocial needs.

The service is focussed on ensuring dementia care in the home is the best it can be and that admission to hospital is avoided wherever possible, considering a person's preferred place of care.
Plan and facilitate individualised care for patients and their carer to improve their quality of life.
Referral to and liaison with GP and multi-disciplinary team or other agencies.
Unfortunately, we are unable to offer the following:
An emergency service
Arrange admissions, scans or clinical investigations
Alter outpatient appointments
Anyone who:
- has a confirmed diagnosis of Dementia is registered with a Warwickshire GP and lives in
- Nuneaton
- Bedworth
- North Warwickshire
- is awaiting an assessment with the Memory Assessment Team
- has been referred by their GP, Consultant or other Health care professional
